Narita international airport is far from Tokyo city area, taking 1.5 hours by a limousine bus or 1 hour by an express train. Around the airport, there are many “Airport hotels with gyms” but they are all in the middle of nowhere (they could not build them close to the airport because of some land disputes back then). All of them have a transportation service to and from the airport. For fitness travelers, there is only 1 local gym selling a day pass. See my recommendations below the map.

Fitness Plaza Breath : Open Tue-Sun 14.00-21.00. Day pass costs $22 per visit. This gym is located within Narita Tobu hotel Airport, and it is available not only for hotel guests but also for non-hotel guests. You can take an airport bus to get to this hotel (gym) for free. The gym facility is old and basic, and free weights are only up to 10kg. It has a shower room. See the official gym website.
Recommendation: This gym is very basic. If you want to have a decent workout before flying out, it is better to do so before going to Narita Airport (while you are still staying in Tokyo or somewhere).

Most of “Airport hotels” in Narita are located in the middle of nowhere with 10-15 minutes bus or taxi ride from the airport. There is really nothing around hotels, but usually hotels have a bus service going to the airport or nearest town for sightseeing (there is a very famous temple in Naria city; Naritasan shinshoji). In this category, my recommendation goes to Hotel Mystays Premier Narita. They have a good gym and hotel guests can use it for free. Other airport hotels charges fees ($10-$30) even to hotel guests. Most of hotel gyms are NOT open 24hrs. You should check the opening hours of the gym if you want to use it early or late. Check out the review and price from here.
For fitness travelers, ANA Crowne Plaza Narita, an IHG Hotel is also a good option. It is relatively close to Narita Airport (5 min by taxi) and has a good gym (and pool). If you are a member of IHG global hotel chain, you can use it for free. If not, it costs $11 per visit
